***Sign on Bonus of $600 for full-time (Half paid out at 3 months and other half at 12 months)*** Salary Range: $52,000 annually Position: We’re looking for a St. Cloud community based direct care case manager that loves working with youth to join our St. Cloud Emergency Youth Shelter! Skilled in trauma-informed care, Mobile Case Managers help youth and young adults work to secure community resources, and plan for the next step on their journey. 180 Degrees, a human services non profit, maintain emergency shelter, transitional living, and supportive housing for youth experiencing homelessness. All of 180 Degrees youth shelters have a mobile case manager that works in direct care with youth exiting shelter, and youth in the community. The work includes direct care and outreach work to build connections with other youth-serving human services agencies. The Shelter Mobile Case Manager will provide short-term direct care crisis response, hold an active caseload, and provide outreach services to the community surrounding the youth shelter. Company: Check us out at: 180degrees.org Care Coordination Responsibilities: - Support the Emergency Youth Shelter in building effective community partnerships and outreach related to the youth experiencing homelessness in the community. - Increase public awareness about shelter services by engaging stakeholders through presentations, meetings, work groups, social media posts, literature drops, and more. - Develop relationships with schools, non profits, librarians, drop-in centers and other community partners that interface with youth experiencing homelessness outside of shelter. - Maintain an updated list of youth-service human services providers and distribute to appropriate parties in shelter and in the community. - Strengthened collaboration to yield new basic center funded referrals. - Provide face-to-face outreach directly to youth in places where they congregate. - Meet individual funding service requirements and documentation for federal/state licensing and grant requirements. - Directly serve youth by managing an ongoing caseload of up to 15 youth. - - Coordinate with appropriate staff and service providers for successful entry into and/or transition out of shelter. - Offer direct care aftercare case management for 90 days to 6 months, providing goal/safety planning, referrals, navigation, advocacy, and financial literacy. - Develop comprehensive case and transition plans tailored to youth and family needs at the non profit. - Facilitate flexible case management through various formats, including face-to-face, text, phone, and virtual interactions. - Identify and refer youth to 180 Degrees human services and other community resources, including a smooth transition to aftercare following discharge. - Document client interactions accurately and promptly, adhering to licensing and grant requirements. - Provide direct care crisis intervention tools, such as food access, transportation options, safety plans, housing advocacy, and financial empowerment for youth not in shelter. 3. Actively contribute to the Youth Shelter leadership team. - - Demonstrate effective teamwork and collaboration with management and colleagues. - Collaborate with supportive human services for youth choosing to not be sheltered that could benefit from an internal referral. - Ensure all services align with best practices for effective youth support in non profits. - Collaborate with the shelter leadership to address support needs, recommend referrals, seek consultation, and establish new processes that better serve youth in aftercare or in need of services. - Engage in the preparation and attendance of case consultations, ensuring inclusion of relevant community partners and shelter staff while respecting data privacy; and reporting on aftercare progress. - Ongoing engagement to bring awareness to youth shelter service and increase of service referrals. - Consult on identifying gaps in services related to referrals, outreach, and aftercare; while being a key stakeholder in implementing an action plan. - Active participant as member of the non profit shelter leadership team, including on-call rotation and youth group shelter programming facilitation. Human Service Competencies: - Committed to the Mission, Core Values, and Vision of 180 Degrees. - Knowledge of homelessness, trauma responsive care, youth-work, and case management best practices; Family and Youth Service Bureau knowledge preferred. - Awareness of one’s own cultural identity, with an understanding of how bias and experience influences their work with diverse individuals. - Thrives on direct engagement with youth, employing skills in safety planning, crisis intervention, emergency services, motivational interviewing and conflict resolution. Qualifications: - Minimum age: 21 - Two years of case management experience related to youth experiencing homelessness or trauma services preferred. - Bachelor’s degree in public health, social work, human services or related field preferred. - Experience in the nonprofit sector preferred. - Dependable transportation for in-person work; reliable internet and phone connection for remote hours. - Valid driver’s license and insurance. - Ability to clear a Department of Human Services background screening. - Lived experience in place of certain credentials will be considered for all positions.
