External Description Str : CentraCare Senior Transitions – Nurse Practitioner or Physician Assistant – Assisted Living Primary Care CentraCare Senior Transitions offers whole-person primary care in assisted living communities, transitional care units, memory care, and long-term care settings. The goal of our provider and nurse teams is to care for patients in whatever senior community you call home. This role will provide mobile geriatric primary care service that offers whole-person primary care in assisted living communities throughout the CentraCare service area. We’re seeking a full-time provider to care for residents in Cold Spring, Watkins, Long Prairie, Paynesville, and St. Joseph. Our team coordinates seamless care by working with building staff, community home care agencies and by offering lab, imaging, and pharmacy support services where and when patients need it to deliver customized care that is proactive and preventative to keep residents in their homes. Our patients experience less ER visits, less hospitalization and less hospital re-admissions compared to patients with similar conditions and complexities. Role Description: - Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities at one community per day - Full-time providers care for patients Monday-Friday. At least 4 hours of dedicated administrative time per week. - Manage a panel of between 130-170 patients (approximately 8-10 patients visits per/day) - Develop and implement care plans appropriate to patients and their desires and geriatric best practices. - Support from scheduling and coordination assistants and clinical support from LPNs and RNs - Access to diagnostics such as labs and x-rays - Collaborate with community partners within home care, hospice, and other resources as appropriate. Schedule: - 1.0 FTE - Monday – Friday – 8am-4:30pm - 1:2 call - One location per day Requirements: - Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program - Board Certified/Board Prepared through AANP, ANCC, or NCCPA - Licensed or ability to obtain licensure through the MN Board of Nursing - Ability to obtain and /or maintain DEA certification for the State of Minnesota - Proven ability to communicate effectively with strong verbal skills - Excellent interpersonal and customer service skills - Must have a valid driver’s license and minimum state-required liability auto insurance; local travel is required CentraCare CentraCare is committed to the patients and families we serve in the communities we call home throughout Central, West Central and Southwestern Minnesota.
