Social Worker | Hospice

External Description Str : Up to $3,000 hiring bonus and $5,000 relocation bonus available for eligible candidates. Find your purpose by helping patients and families through the end of life. This role requires intense dedication and provides immense reward. You’ll help patients and families understand options and help guide them to decisions. The work can be heavy and yet it will lift you up. This position primarily serves home health and hospice patients in St. Joseph, Sartell, St. Stephen, Holdingford, Avon and surrounding areas. So, you’ll travel but just locally. You'll collaborate with a large team of other Social Workers to gain knowledge and experience and work with an interdisciplinary team of nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources. Schedule: - Part-time - 40 hours every two weeks. - Monday-Friday 8 a.m.-5 p.m. - No weekends or holidays! - Possibility to flex up and pick up additional shifts when available. Qualifications: - Current Licensed Social Worker (LSW) in Minnesota required. - Bachelor's degree in Social Work or Sociology. - At least two years of experience as a Social Worker, preferably in a healthcare setting. - One year of home care or hospice social work experience preferred. - Reliable transportation required. - Valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage required. Hiring bonus requirements: To be eligible for the sign-on bonus you must be an external candidate who is hired into an eligible full time or part time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire. Relocation assistance requirements: Must accept a position that offers benefits and may not have worked for CentraCare in the last 9 months. Must currently reside more than 50 miles away from St. Cloud and relocate to within 30 miles of it.
