At the St. Cloud Hospital, our Socials Workers serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You will be able to collaborate with a large team of other Social Workers to gain knowledge and experience while also working with an interdisciplinary team made up of nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources. This Social Worker position will support the Medical/Oncology Units at the St. Cloud Hospital. Schedule: - Part-time, 48 hours per two week pay period, Monday-Friday, 8am-4:30pm Qualifications: - Current and valid LSW in MN required - Bachelor's degree in Social Work or bachelor's degree in Sociology with 5 years of Social Work required - American Case Management Association (ACMA) Case Manager-SW Certification or obtain within 5 years of hire required CentraCare Health and Carris Health have made a commitment to diversity in its workforce.
