To provide professional case management to individuals who are eligible for CAC/CADI/BI Waiver services, Developmental Disability (DD) Waiver Services, and/or Rule 185 (Development Disability) case management. Mandated services are provided as required by Federal and State Statute and Rules. To develop and coordinate person centered plans with individuals with disabilities, their families and support network. To provide direct services of counseling, facilitation of interdisciplinary team meetings, coordination and consultation that address the bio-psychosocial well-being of the individual that remain in compliance with County, State and Federal Rules and Statutes. This position is located at the Government Center in Elk River, MN. Our regular business hours are Monday-Friday 8:00am-4:30pm, though the department provides staff with flexibility in effort to strive for a desired work/life balance.
