Social Worker Location: Guardian Angels Care Center – Elk River Job ID: 2645 Benefits to Working at GA: - 10 Holidays - Monthly Wellness Initiatives - Employee Support Program - Collaborative Work Environment - Supportive Team Culture - Free Gym Membership – Club GA & Gym 365 Schedule: - Monday - Friday - Part time, working 64 hours per pay period (2 Weeks) - Benefit Eligible Position Summary: The Social Worker assists the Director of Social Services in planning, directing, and organizing the social services program to meet the objectives of the care center as part of total resident care. This role is primarily working within the Transitional Care Unit . Services are provided in accordance with the mission of Guardian Angels Care Center and in compliance with existing rules and regulations. Job Duties: - Assist in the admission process, including completion of admission paperwork. - Actively assist with discharge planning starting on admission with transitional care residents. - Complete a social history and psychosocial assessment for each resident that identifies social, emotional, and psychological needs. - Complete the MDS, CAAs, BIMS, PHQ-9. - Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident, the goals to be accomplished for those needs/issues, and the appropriate social worker interventions. - Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility, including their social, emotional, and psychological needs. - Will communicate and participate in interdisciplinary team meetings. - Provide support to team for identification of mood and behavioral expressions. - Participate in the assessment of the appropriateness of referrals for nursing home placement/assisted living. - Act as advocate for the residents and assist them in maintaining a satisfactory lifestyle in the Care Center. - Assist in referrals and coordination with psychological services. - Assist in providing a dignified, respectful, resident-centered environment for residents. - Ensure or provide support and education to residents/family members/significant others to assist in their understanding of placement and facility issues in addition to referring them to the appropriate social service agencies when the facility does not provide the needed services. - Provide groups for residents/family members/significant others as appropriate to their needs. - Provide interventions to address traumatic events that have occurred to resident. - Coordinate the resident discharge planning process and make referrals for appropriate home care services prior to the resident’s return to the community - Follow standard precautions, facility infection control guidelines, sanitation and safety standards of practice in all work and activities. - All duties are performed in accordance with current standards, guidelines, state and federal regulations. - Uphold confidential information as required by Guardian Angels Care Center policy and procedure and as related to HIPAA. - Uphold the mission, vision, and values of the Guardian Angels of Elk River. Qualifications: - Current Minnesota Social Worker License preferred, not required. - Previous work in Long Term Care setting preferred. - Must be self-motivated, with ability to organize and prioritize. - Must have excellent verbal and written communication skills. - Must have ability to collaborate with other health care professionals, institutions, and agencies.
