Duties The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. RN duties include but are not limited to: - Demonstrates the ability to provide nursing care based on utilization of the health history and physical assessment skills and provides assistance for less experienced staff. - Demonstrates knowledge of Code Green Policy and Procedure and provides leadership for less experienced staff. - Maintains appropriate intervention of 1:1 observation and provides leadership for staff - Conducts a comprehensive assessment of the patient's health and psychosocial needs, including health literacy status and deficits, and develops a plan collaboratively with the patient and family or caregiver. - Plans with patient, family or caregiver, the specialist, the PACT team, other health care providers, and the community to maximize health care responses, quality, and cost-effective outcomes. - Facilitates communication and coordination between members of the health care team, involving the patient in the decision-making process in order to minimize fragmentation in services. - Educate the patient, the family or caregiver, and members of the health care delivery team about treatment options (including but not limited to CVT, HT, evidenced based protocols), community resources, psychosocial concerns, and case management, so that timely and informed decisions can be made. - Empower the patient to problem-solve by exploring options of care and alternative plans to achieve desired outcomes. . Encourage the appropriate use of health care services and strives to improve quality of care and maintain cost effectiveness on a case-by-case basis. - Assist the patient in the safe transitioning of care to the next most appropriate level. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Physical Requirements: This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling and pushing. Transferring patients and objects may be required. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
