As a Physical Therapy Assistant (PTA) / Occupational Therapy Assistant (COTA) at Champion Rehab, you'll:+ Provide direct patient care: continuous assessment of patient needs and functional deficits, patient care/treatment, ongoing modifications to the existing plan of care as needed, ability to initiate appropriate referrals to other disciplines when needed + Work closely with a skilled and dedicated interdisciplinary team to maximize patient satisfaction and overall success based on diagnosis and comorbidities+ Apply clinical expertise as necessary including but not limited to family and staff education/training, recommendations and guidance for safe and successful discharge, communicating with Physicians to ensure fluency of care+ Complete documentation which follows regulatory guidelines (state, federal)Qualifications for COTA and OTR Therapist:+ Graduate of an accredited program+ Currently licensed to practice in the State of MN.
