Help Help Duties The Medical/Family APN is responsible for the planning, integrating, implementing, modifying and evaluating the effectiveness of the Veteran's care. The MH Residential Rehabilitation Treatment Program and the Psychiatric Acute Unit foundational elements will serve to support delivery of high quality, timely care to the Veteran receiving services. The APN is the lead of the integrated care team established by the MH Residential Rehabilitation Treatment Program and the Psychiatric Acute Unit. He/she is responsible for providing clinical leadership and expertise on Mental Health Integrated Clinical Community processes and regulations. The APN will serve as a liaison to both internal and external providers and will be responsible for managing the care of the Veteran and the seamless transition to and from the community. He/she possesses the knowledge related to VHA guidelines, utilization review criteria, current evidence based standards of care, compliance guidelines and local policy related to Delegation of Authority as established by the facility Chief of Staff. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs. The nurse's primary commitment is to the patient, whether an individual, family, group, or community. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Preferred Experience : 2-3 Years Medical/Family Nurse Practitioner experience Physical Requirements: Heavy lifting (45# or more), moderate carrying (up to 45#), reaching above shoulders, use of both hands and legs, use of fingers, prolonged walking and standing, intermittent crawling, kneeling, and bending, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, far binocular correctable to 20/40, vision, hearing (aide permitted), mental and emotional stability. Additional information Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
