Position Summary: The LPN is responsible for the observations, care and services provided to assist residents. Plan, assign, and direct work of assigned staff. Maintain residents’ personal dignity, self-respect and safety at all times. Responsible for work supplies and tools to ensure that resources are used wisely. Supports and implements all safety practices. Job Duties: - Works collaboratively with RN/Home Care Director to coordinate Resident Care. - Conducts meetings/reports to communicate Resident Care concerns/issues. - Participates in care conferences as needed. - Monitors physician orders, medications and flow sheets for accuracy. - Assists with the implementation of physicians' orders. - Implements proper charting procedures. - Consult with RN/Home Care Director regarding resident conditions and treatment plans. - Assists with admissions, transfers and discharges as requested by RN/Home Care Director. - Proficient operation of medical equipment. - Ensures that unlicensed staff assignments are completed in a correct and timely manner. Reports concerns to RN/Home Care Director - Report all changes in the resident’s condition to RN/Home Care Director. Initiate emergency treatment if needed. - Assists with weekly medication management for administration by unlicensed staff. Reorder medications as needed. - Properly documents all medication disposal per policy/procedure. - Plan, assign, and direct work of unlicensed staff according to established guidelines. - Communicates resident information to staff via 24 hour communication book. - Attends and completes required in-services, meetings and conferences. - Assists unlicensed staff as needed. - Performs Indirect Supervisory visits as assigned. - Follows all aspects of Minnesota Nurse Practice Act. - Is available for on-call medical questions/emergencies via phone, consults/updates RN/Home Care Director as required by licensure. - To assist in providing a dignified, respectful, resident-centered environment for residents. - Follows universal precautions, facility infection control guidelines, sanitation and safety standards of practice in all work and activities. - All duties will be performed in accordance with current standards, guidelines, state and federal regulations. - Uphold confidential information as required by Guardian Angels policy and procedure and as related to HIPAA. - Uphold the mission, vision, and values of Guardian Angels. - Performs all other duties as assigned. Qualifications: - Current license as an LPN in the state of MN required. - CPR certification is required. - Three to five years prior nursing experience is preferred. - On-Call rotation is required. - Excellent verbal and written communication skills. - Self-motivated, ability to prioritize.
