A licensed practical nurse is a clinically competent position that provides care to a group of home health patients/residents under general supervision/delegation of a registered nurse according to established policies, procedures, protocols and standards of practice and care. Effectively interacts with patients/residents, families, responsible party, and all health team members to assure resident safety. - Perform initial data collection on all patients/residents and follows up on data collection as per policy. - Participate in implementing person driven plans of care as developed by the Registered Nurse within established policies and procedures and continually works to meet individual patient/resident needs. - Demonstrate knowledge of medications and their correct administration based on age of the patient/resident and their clinical condition. Observe, document and report patient/resident responses to medications. - Follow patient/resident, family and caregiver education plan of care as developed by the Registered Nurse, clearly communicates information and documents patient/resident response to education. - Document according to current standards and policies. - Plan for patient/resident discharge, collaborates with the Registered Nurse and discharge planners for long and short term patient/resident needs. - Coordinate and direct patient/resident care appropriately. - Perform charge nurse or building responsibilities when assigned. - Utilize the principles of Relationship Based Care while performing duties. - Promotes the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into the day-to-day activities of the company. - Performs other duties, tasks and/or projects as assigned. QUALIFICATIONS REQUIRED Current state licensure as a Practical Nurse, which must be in good standing BLS certified Valid Class A Driver’s License QUALIFICATIONS PREFERRED Clinical experience EEO/AA/Vet Friendly $26.00-$26.78
