Nystrom & Associates is currently seeking a Case Manager to join our Behavioral Health Home team as Systems Navigator (BHHSN)! This BHHSN will provide client-centered, family-focused, culturally sensitive, community-based case management services to clients and their families. Primary duties include ensuring individuals and their identified supports have access to medical, behavioral health, pharmacology, and recovery support services. In addition, the Behavioral Health Home Systems Navigator will: - Provide care coordination with clients and their identified supports, including medical, behavioral health and community, or county providers. - Ensure that the client is aware of resources and are supported in efforts to access resources in order to address the client’s identified goals and needs (i.e., county social services, housing, employment). - Assist as needed in conducting the initial needs assessment and developing the preliminary action plan. - Monitor client progress on goals in the care plan and identify possible needs for modifying/updating the care plan. - Work collaboratively with Registered Nurse and Peer Support Specialist to ensure best possible client outcomes. - Maintain legible and organized patient electronic health records in accordance with policy of the company and requirements of the State of Minnesota. Concurrent documentation is practiced - Attend regularly scheduled staff and training meetings - Other duties as assigned In order to be considered for this position, you must meet the qualifications of a Mental Health Practitioner OR Certified Community Health Worker (CHW) as listed below: Mental Health Practitioner Qualifications - Master's degree or other graduate degree in one of the behavioral sciences or related fields from an accredited college or university - Bachelor's degree in one of the behavioral sciences or related fields AND 2,000 hours of supervised experience in the delivery of clinical services in the treatment of mental illness or children with emotional disturbances - 4,000 hours of supervised experience in the delivery of mental health services to recipients with mental illness Community Health Worker Qualifications - Must have completed a DHS approved course and recieved certification. Please note that Nystrom does NOT provide CHW certification courses.
