Description Licensed Social Worker (LSW) Location: Bridge Recovery - Sauk Rapids, MN Meridian Behavioral Health LLC is a leader in behavioral health care and addiction recovery services, with a network of addiction treatment centers. We are opening a new men's residential location in Sauk Rapids named Bridge Recovery. We are looking for passionate, committed individuals to join our team. We are looking to add a LSW to our clinical team to support group education needs. Job Duties: + Initiate and complete all relevant documentation, i.e., group attendance records, group notes, progress notes, master treatment plan, communication log, etc. within required timeframes + Demonstrate ability to adjust own workload to meet fluctuation in patient census + Able to identify significant changes in patient's condition and take appropriate actions as evidenced in reporting, staffing consultations, and documentation + Participates in a rotation of evenings, weekends, and holidays + Perform related duties as required, including but not limited to group facilitation and patient care + Serve as part of multidisciplinary treatment team to develop and implement individualized and measurable treatment goals and modalities for each patient Job Requirements: + Bachelor's degree in Social Work is required + Hold current Social Work license in Minnesota.
