Job Description Full-time 80 hours; Monday-Friday 8am-5pm SUMMARY Responsible for completing a focused comprehensive health assessment, including physical, psychological, cultural, and social components, to diagnose patient problems, identify outcomes, plan, implement, and evaluate patient care. Collaborates with the multidisciplinary team through Relationship-Based Care, to support a healthy work environment and ensure a culture of safety. Assesses patient at a minimum monthly at patient's clinic visit and prn. Proper referrals made to Social Services, Nutrition Services and Clinical Resources, etc. Receive and implement physician orders of dialysis nature and refers non-nephrologists orders appropriately. EDUCATION Graduate from a nursing program required. Baccalaureate or Master's in nursing preferred. A minimum of a Baccalaureate degree in Nursing must be obtained within three (3) years for those hired into an RN position on or after October 1, 2018. EXPERIENCE One (1) year clinical experience, with 3 months providing care to patients on maintenance dialysis, self-care and home training. Knowledge of nursing principles, policies, procedures, and hospital ethics required. SPECIAL REQUIREMENTS Current licensure as a registered nurse in the State of Minnesota required. Current American Heart Association (AHA) BLS Healthcare Provider card required per established policy. Nephrology nursing certification preferred. American Nephrology Nurses Association (ANNA) membership preferred. A valid MN driver's license, a driving record in good standing and minimum amount of insurance liability coverage required.
