Job Summary To provide MnCHOICES assessment services to the elderly and disabled population to determine eligibility for State Waiver programs, PCA, and Rule 185 Case Management. To develop and coordinate a person-centered support plan with individuals, their families, and support network. To provide direct support to individuals that address the bio-psychosocial wellbeing of the person that remains in compliance with county state and federal rules and statutes. To provide mandated services as required by statute. Regular attendance according to the department's approved work schedule is required for all Sherburne County positions. Carry out the responsibilities of the lead agency for conducting MnCHOICES assessments to determine eligibility for State funded programs. Complete person-centered Community Support Plans that reflect the needs of the individual that were identified through the assessment process. Participate in regularly scheduled interdisciplinary team meetings to review service eligibility. Complete assessments, receive referrals for services and determine eligibility Complete referrals as appropriate Ensure data is kept confidential per HIPAA and data privacy standards. Provide outreach services to target populations.
