Applicants must have a Bachelors Degree in social work or related human services field and Two (2) years working with children and adolescents doing individual counseling, group work and case coordination. Job Summary: The Independent Living Skills Counselor is responsible for providing independent living development services one on one to youth between the ages of 16-24 in the SHY scattered site program and the Youth House. The Independent Living Skills Counselor implements independent living plans approved by the Supervisor in conjunction with the referring agency. They also provide individual case management and coordinate with referring workers. ILS counselors also may facilitate independent living skills groups in the community. Major Job Responsibilities: - Provide case management to at-risk youth/young adults on a weekly basis, establishing Independent living plans based on the youth’s goals & monitoring their success, responding to resident crises as necessary, and, advocating for and teaching youth to advocate for themselves. At times, may require work be completed at client’s home/other community venue. - Intermittently provide group facilitation for ILS groups in the community teaching independent living skills. - Complete required client assessments, progress notes, Fed/state admission forms, and additional reporting/miscellaneous documentation for funding organizations or licensing boards. - In order to provide services to clients, position requires ILS counselor maintain professional relationships & constant communication with County Social Service counselors/case workers, mental health professionals, probation officers, property managers & other service providers and other community resources. - Enhance the professionalism in the position by participating in group case consultation, attending professional development trainings, and other offered dept. opportunities.
