We are currently seeking full-time or part-time Mental Health Practitioners for our in-home Adult Rehabilitative Mental Health Services (ARMHS) program! ***Option to meet with clients via telehealth until at least June 2021!*** Family Support Services is a division of Nystrom & Associates that provides ARMHS, Adult Rehabilitative Mental Health Services, programming sponsored by the Minnesota Department of Human Services for individuals who have a mental health diagnosis and are on Medical Assistance or a Prepaid Medical Assistance Product. The purpose of the program is to help individuals who have a mental illness function independently in their homes or places of residence. Services are provided in four areas: Basic Living and Social Skills, Community Intervention, Medication Education and Transitioning to Community Living. Mental Health Practitioners maintain a full-time or part-time caseloads of 8-20 clients, build relationships with clients, implement rehabilitative mental health services, including but not limited to: basic living skills, social skills, and community intervention services. Other responsibilities include: - Assist clients in identifying goals for functional improvement and work with them to meet their goals - Design and implement treatment/intervention in-home reflective of client needs, following all Department of Human Services related standards - Maintain legible and organized patient electronic health records in accordance with policy of the company and requirements of the State of Minnesota.
