Position Summary The purpose of this position is to assist people with serious mental illness live independently in their own homes. Responsibilities - Communication: Speak clearly, listen to others, write effectively, establish relationships, act with integrity, and establish trust. - Build and maintain effective working relationships with site contacts and maintain business partner’s expectations for productivity, quality, etc. - Provide individualized housing support services through regular meetings with persons served to work on independent living skills. - Assist persons served with budgeting, self-sufficiency planning and community integration. - Responsible for maintaining a caseload of 15-20 people while tracking and documenting variety of data about services provided as well as billing through electronic case management systems. - Utilize person centered thinking, motivational interviewing and mental health symptomology to best engage with persons served. - Triage incoming calls to determine eligibility / track data elements. - Outreach to persons least likely to access service or know about resource. - Outreach program details to local agencies and community partners. Requirements - Degree in human service field and experience working with adults with a mental health diagnoses, providing mental health services. - Excellent time management and multi-tasking skills with strong ability to problem solve. - Technology skills: Proficient with Microsoft Office software (Outlook, Word, etc.). Case management (Care Director) – Learn software system and become proficient. - Applicants must have a valid driver license. - A driving record that demonstrates safe, legal driving habits. - Applicants must comply with and pass Department of Human Services background check (Rule 11). - Must be able to pass a Motor Vehicle Record (MVR) background check.
