Overview Fairview Health Services and HealthEast is searching for a Diabetes Education Specialist (RN or RD) to support the Fairview Clinics Eagan location with potential for additional coverage of one or two additional southwest metro locations (Burnsville, Prior Lake, Savage, Lakeville, Rosemount, Farmington clinics). This is a 1.0 FTE (80 hours per two week pay period), benefits-eligible position. This position requires current CDE (Certified Diabetes Educator) certification. The Certified Diabetes Education Specialist is responsible for the education of patients, their families and appropriate support systems about diabetes self-management and related issues. The CDES provides diabetes self-management education (DSME) and coordinates protocol-based diagnostic testing and treatment, which may include medication management, nutrition, physical activity, healthy coping, risk reduction, monitoring and problem-solving. The CDES also serves as a resource to other staff members as needed. Provide consultation to providers on best practice in management of patients with diabetes. Following specific curriculum developed for diabetes education - Assesses patient's physical, psychosocial, behavioral and educational needs (to include self-management, nutrition, physical activity, medication taking, and health maintenance labs) and interprets the information to identify individualized clinical and educational needs. - Teaches technical stills, concepts of diabetes self management and facilitates behavior change for patients with Type 1, Type 2, and gestational diabetes as well as those with pre-diabetes. - Collaborates with the patient to establish individual goals. - Communicates with providers and nursing staff to keep them aware of patient progress and recommends changes in regimen as appropriate. - Evaluates the patients comprehension of completed education and develops follow up plans to address ongoing needs. - Documentation of required assessments, interventions, evaluations, and follow-up is consistent with established departmental and ADA standards. - Individualizes education presentation & materials to the patient and patient circumstance. Qualifications Education Required: Associate's Degree in Nursing or Dietetics Preferred: Bachelor's Degree Licensure/Certification Required: Registered Dietician (RD-Commission of Dietetic Registration) and License Dietician (LD-MN Board of Dietetics and Nutrition Practice) or Registered Nurse MN (RN-MN Board of Nursing) Current Certified Diabetes Educator (CDE) certification is required
