The Advance Care Planning Program Manager will work closely with the internal stakeholders and community partners to lead multi-disciplinary system and community wide advance care planning initiatives. This position is responsible for organizing, promoting, implementing, and supporting CentraCare advance care planning strategies throughout the CentraCare service area. This includes, but is not limited to, working with providers, health care staff, community members, and business and community leaders. Schedule: - Full-time, 64 hours per two week pay period, Monday-Friday, 8am-4:30pm Qualifications: - Bachelor’s degree in Public Health, Business, Communications or Marketing required - Experience in successfully implementing policy, system and environmental changes - Previous experience with community organizing and relationship building - Demonstrated skills including leadership, problem solving, communication, organization and goal setting - Creativity is necessary for planning special events and program material development CentraCare Health and Carris Health have made a commitment to diversity in its workforce.
